These are two tiers designed for two levels of complexity of care:
Tier 2

Patients on 4+ medications or requiring insulin management.

Requiring medical optimization

Polypharmacy (>4 routine meds).

Condition instability requiring active titration.

Need for Remote Patient Monitoring (RPM).

RPM Integration: Review and management of data from home devices (BP cuff, Glucometer, Scale). Note: Devices at no additional cost because our priority your benefit
