Program participation: Active enrollment in Medicaid, SNAP/Food Stamps, WIC, Lifeline/ACP, or a Silver Marketplace plan with Cost-Sharing Reductions. How to Enroll: Click "Book Now" and select the "Low Income Tier".Complete the intake forms sent to your email.Check the boxes for the criteria you meet. ⚠️ Important Privacy Notice: Do NOT email us your proof of income. To protect your security, you will upload your documentation directly through our secure, HIPAA-compliant patient portal after booking. Note: To keep this tier affordable and available to the Florida families who need it most, we operate on a shared-boundary model. This allows us to keep our overhead low and dedicate focused, high-quality time to your health when you truly need it.
